Overview
This unit will provide you with a broad understanding of the application of safety processes within the aviation industry. You will learn basic concepts of system safety as they developed alongside the aviation industry. You will also learn about standards and practices for safety management with reference to the International Civil Aviation Organization (ICAO) and CASA. You will recognize a systematic approach that is established using four main components including Safety Policy, Safety Assurance, Safety Risk Management, and Safety Promotion to manage safety and how human factors are related within the four components of safety. You will also develop a solid understanding of safety culture and its associated subcultures.

There are no requisites for this unit.
Important note: Students enrolled in a subsequent unit who failed their pre-requisite unit, should drop the subsequent unit before the census date or within 10 working days of Fail grade notification. Students who do not drop the unit in this timeframe cannot later drop the unit without academic and financial liability. Attendance Requirements
All on-campus students are expected to attend scheduled classes - in some units, these classes are identified as a mandatory (pass/fail) component and attendance is compulsory. International students, on a student visa, must maintain a full time study load and meet both attendance and academic progress requirements in each study period (satisfactory attendance for International students is defined as maintaining at least an 80% attendance record).
Recommended Student Time Commitment
Each 6-credit Undergraduate unit at CQUniversity requires an overall time commitment of an average of 12.5 hours of study per week, making a total of 150 hours for the unit.

This is a graded unit: your overall grade will be calculated from the marks or grades for each assessment task, based on the relative weightings shown in the table above. You must obtain an overall mark for the unit of at least 50%, or an overall grade of 'pass' in order to pass the unit. If any 'pass/fail' tasks are shown in the table above they must also be completed successfully ('pass' grade). Writing Style
Students are recommended to purchase the Manual of Publication by the American Psychological Association (APA) (7th Ed.). It is the major reference source for style or format questions in aviation program. The format of references, citations, quotation, and headings should be identical to APA requirements. Be sure to read and understand the plagiarism sections.

There are 3 quizzes throughout this unit:
- Quiz 1 will be conducted at the end of Week 2, covering basic safety concepts and SCM.
- Quiz 2 will be conducted at the end of Week 4, covering human contribution to accidents and performance concepts.
- Quiz 3 will be conducted at the end of Week 8, covering safety culture and safety subcultures.
You have 30 minutes to complete each quiz, and only one attempt is permitted. Each quiz will be available on Moodle from Monday evening of the scheduled week and must be submitted by Friday at 5:00 PM (AEST) via the relevant portal in Moodle.

2 Case Study
Overview
You are required to choose an airline or airport accident to research and analyze using the Swiss Cheese Model. You may work individually or collaborate in pairs (maximum of two people per group). This project involves two components:
- Presentation: Prepare a 5~10-minute presentation on your case study to be delivered to the class either in person or via Zoom during Week 5.
- Written Report: Submit a comprehensive report detailing your analysis.
Report Requirements:
- You must complete your report within the MyStudy portal of Moodle, which includes a template that you must follow.
- Length: The report should be between 5–10 pages, excluding the reference list, figures, and tables.
- Format: Follow APA formatting guidelines, such as:

Presentation Requirements
- Your presentation must adhere to the 5–10 minute time frame. Presentations exceeding the time limit by over one minute will incur a 10% grade deduction.

The overall objective is to accurately apply the Swiss Cheese Model, including the identification of active failures and latent conditions.
Your Case Study Project will be evaluated based on both the presentation and the written report. The total available points for this assessment are 30 points, allocated as follows:

3 Online Test
The final test will be conducted during the exam week and will cover Safety Management Systems and Safety Culture concepts discussed in Weeks 7–12.
You have up to 2 hours to complete the test, and only one attempt is permitted. The test will open on Monday evening in exam week and must be submitted by Friday at 5PM via the relevant portal in Moodle.
Instructions:
- Accessing the Test: The test will be accessible through the designated portal on Moodle starting Monday evening of the exam week.
- Submission Deadline: Ensure you submit your completed test by Friday at 5:00 PM (AEST) of the exam week. Late submissions will not be accepted.
- Technical Requirements: Make sure you have a reliable internet connection before starting the test.
- One-Time Entry: Once you begin the test, you must complete it in one sitting within the 2-hour time limit.

This test is a combination of multiple choice and short-answer questions. Review all materials and concepts from Weeks 7–12 to ensure you are well-prepared.
