General Information

Overview

This unit will introduce you to concepts and knowledge required to effectively care for patients experiencing complex alterations in health. You will critically consider the care provided to patients, including strategies to assist individuals and significant others when dealing with health crises. In addition, effective communication and collaboration in a complex situation is paramount. Appropriate provision of care including assessment, planning, implementation and evaluation of safe nursing care in various complex health care situations will be adopted in this unit. You will be expected to function within the scope of practice of a nursing student in accordance with the Nursing and Midwifery Board Australia (NMBA) registered nurse standards for practice during the compulsory three (3) day residential school or timetabled internal nursing laboratory sessions and 160 hour clinical placement consisting of 80 hours of complex care and 80 hours of mental health setting.

Attendance Requirements

All on-campus students are expected to attend scheduled classes – in some units, these classes are identified as a mandatory (pass/fail) component and attendance is compulsory. International students, on a student visa, must maintain a full time study load and meet both attendance and academic progress requirements in each study period (satisfactory attendance for International students is defined as maintaining at least an 80% attendance record).

What are the best aspects of your unit? Students enjoyed CNP3 on-campus learning experiences. In particular, many students praised the quality of the lecturers and felt supported. The hands-on experience in the labs prepared the students well for the clinical placements.

CQUniversity staff will continue to utilize best learning and teaching practices aimed at fostering and developing a supportive learning environment for all students.

Feedback

What aspects of your unit are most in need of improvement? A number of students commented on the on-campus clinical assessments, saying that they felt there was an inconsistency between the assessments (3 options) and that they did not enjoy playing the role of the patient in the assessments.

Recommendation

On-campus clinical assessments will be reviewed to ensure consistency and the role of the patient in future assessments will be played by simulated patients, not students.

What aspects of your unit are most in need of improvement? Students expressed dissatisfaction with the number of written assessments required to meet the course criteria.

Recommendation

The number of written reflections will be reviewed and reduced for future iterations of CNP3. This will be possible without the need for formal change requirements as the inclusion of 3 of the 4 existing written reflections have been superfluous to the unit assessment task requirements.

Clinical Placement begins for some students

It is therefore highly recommended that prior to attending the first on-campus learning session that you revise and practice medication calculations using the Med+Safe Learning Program (links on Moodle).

Term Specific Information

Recency of Practice

To attend a clinical placement, CQU students must meet recency of practice (ROP) criteria. Meeting the ROP criteria reassures our industry partners (the clinical settings where you are going on placement) that CQU students are equipped and ready to work safely within the scope of practice of a second year nursing student (as outlined in your course material). We must, therefore, establish who meets the recency of practice criteria and who does not, prior to all students commencing their placements. If you have been practicing as an Enrolled Nurse (medication endorsed) within the previous 12 months, then you meet the recency of practice criteria. However, if you are an EN who has not practiced within the previous 12 months, then you do not meet the recency of practice criteria and you will need to visit the Work Integrated Learning Moodle site and review the ROP requirements.

If you have completed a clinical placement as a component of a Bachelor of Nursing degree within the previous 12 months, then you meet the recency of practice criteria.

If you have not completed a clinical placement within the previous 12 months, you do not meet the Recency of Practice criteria and will need to visit the Work Integrated Learning Moodle site and review the ROP requirements.

Please note that if you are a Recency of Practice student returning to the Bachelor of Nursing after a break of more than 23 months you may be required to attend further clinical learning sessions prior to commencement of NURS12158.

Task Description

CNP3 Assessment 1 - Practical and Written Assessment.

Assessment 1 has practical and written components designed to assess your ability in a number of clinical skills while at the NURS12158 Clinical Nursing Practice 3 on-campus learning sessions. Whilst attending these on-campus learning sessions you will have the opportunity to refine the theoretical knowledge and practical application of clinical skills in the clinical learning space. You will undertake practical and written clinical skills assessments throughout your on-campus learning experience. These assessments are designed to identify and confirm that students are equipped and ready to work safely within the scope of practice of a second year nursing student (as outlined in your course material) prior to undertaking clinical placement.

  • The practical component of Assessment 1 involves a simulation-based assessment focusing on your clinical skills and safe nursing practice within the scope of practice of a student undertaking NURS12158.
  • The written component of Assessment 1 requires you to write patient progress notes, using a SOAPIE format, to detail the care you provided for the patient in the simulation assessment scenario.

Assessment Criteria

CNP3 Assessment 1 - Practical Component – Simulation Assessment.

Practical Clinical Skills Simulation Assessment will be based on skills covered during the on-campus clinical learning sessions. To attain a pass you must achieve all the following components for each skill:

  • Student demonstrates the 5 moments of hand hygiene and infection control principles.

  • Student safely assesses situation and person as per simulation scenario.

  • Student demonstrates safe and timely administration of medication adhering to the “5 rights of medication
    administration”, 3 checks and 3 identifiers.

  • Student undertakes clinical procedure as per simulation scenario with particularly emphasis on patient safety,
    critical thinking and team work.

  • Student effectively communicates with patient in a person centered approach.

  • Student demonstrates application of the ISBAR communication technique.

  • Student applies legal principles of professional documentation in bedside chart.

Please note: While the clinical skill assessment marking tool is provided to students prior to their clinical learning session to assist in learning the marking tool cannot be taken into the assessment. All assessments in this unit are assessed as per the Assessment Policy and Procedure (Higher Education Coursework).

**100% attendance (24 hours in total) and participation in the clinical simulation assessment is required to PASS this assessment. ***

Important: A maximum of three (3) attempts at completing the Practical Assessment of Clinical Skills will be permitted. Should you not achieve the required standard by the third attempt you will not be eligible to complete the clinical placement component of NURS12158 and the CQUniversity Work-Integrated Learning/Student Placements policy and procedures will be enacted.


 CNP3 Assessment 1 - Written Component – Patient Progress Notes (using SOAPIE)

The written component of assessment 1 requires you to complete patient progress notes, using the SOAPIE format, for the patient you provided care to in the simulation activity. To attain a pass you must achieve all the following components of the progress notes:

  1.  Entry has nursing as title with date and time is clearly recorded.
  2. Subjective data clear identification of nursing entry
  3. Objective data clear in progress notes.
  4. Assessment data/information able to be identified in nursing progress notes.
  5. Plan of action clear in the nursing progress notes.
  6. Implementation of patient care identified and explained in nursing progress notes.
  7. Evaluation clearly articulated in nursing progress notes.
  8. Nursing progress notes correctly signed with printed name and designation (i.e student nurse CQU or SRN CQU)
  9. Spelling correct and any ‘error’ correctly identified with a single line, word error and initial.

 Please see the Unit Moodle site for the detailed marking guide to be completed by you and uploaded along with your progess notes. This assessment item must be uploaded to the Moodle site within three days of completing the practical component (the clinical simulation assessment).

Task Description

CNP3 Assessment 2 – Written Assessment - Medication safety quiz

You will be required to undertake an invigilated medication safety examination using the Med+Safe Learning Program software on the first day of your on-campus learning sessions. You may be assessed on any level of medication safety that you have previously covered in the Bachelor of Nursing program at CQUniversity, or for ENs joining the unit this term, medication calculations included in the expected scope of practice of an Enrolled Nurse.

You are required to achieve 100% in the medication safety examination in order to demonstrate safe nursing practice. You are required to upload your certificate of successful completion to Moodle immediately after the quiz, prior to leaving the examination area. You will have three (3) attempts only at undertaking the medication safety examination. If you score less than 100% in your first attempt, you will be provided with an additional two (2) opportunities to achieve a Pass. The second attempt will follow immediately after the first attempt and the third attempt will be prior to Day 3 of your on-campus learning sessions. If you are not successful on this third attempt, you will not be able to proceed to clinical placement and will be a Fail for the unit.

3 Professional Practice Placement

Clinical Placements x 2 (80 hours of Complex care AND 80 hours of Mental Health)

Task Description

CNP3 Assessment 3 – Clinical Placement

The NURS12158, 2019 Clinical Competency Performance Assessment requires the successful completion of two clinical placement components. One placement component is complex care which requires 80 hours. The other is a mental health placement component which also requires 80 hours. A total of 160 clinical hours are required for this course. You will demonstrate your ability to perform the nursing role within your NURS12158 scope of practice as measured against the NMBA Registered Nurse Standards for Practice outlined in the 2019 Competency Performance Assessment (ANSAT) document.

If you do not receive a pass for both the Mental Health AND the Complex Care clinical placement you will not be eligible for supplementary assessment and will receive a fail grade for NURS12158 Clinical Nursing Practice 3.

  • You are required to submit your formative self-assessment and your assessor’s formative assessment at the midway point of both your clinical placements.
  • At the end of each of your clinical placements, you are required to submit your completed 2019 Clinical Placement Australian Nursing Standards Assessment tool (ANSAT) documentation, including your assessor’s summative assessment, your signed record of attendance and if applicable, your completed and signed support plan. These documents need to be uploaded to the course Moodle site within seven days of completing each clinical placement.

Assessment Criteria

CNP3 Assessment 3 – Clinical Placement

The 2019 Australian Nursing Standards Assessment tool (ANSAT) has been designed to enable you to demonstrate the development of professional attributes required by the registered nurse. The criteria in the ANSAT is based on the NMBA Registered Nurse Standards for Practice. These criteria will be used to evaluate your ability to perform your nursing role within your scope of practice and to determine your ability to meet the NMBA Registered Nurse Standards for Practice as per your NURS12158 level. For example, as you are a NURS12158 student it is expected that you will perform within the scope of practice of a NURS12158 nursing student and demonstrate critical thinking, problem solving and clinical skills applicable for a NURS12158 nursing student.

Assessment criteria and performance standards for the 2019 Australian Nursing Standards Assessment tool includes:

  • · completion of the required hours for each clinical placement (80 hours in complex care and 80 hours in mental health, equalling a total of 160 hours) as stipulated by the relevant unit coordinator;
  • · a self-assessment of your own clinical performance against the NMBA Registered Nurse Standards for Practice at the formative stage of the clinical placement, which you are required to upload at the midway point of each placement; and
  • · assessment and recording of your performance by the identified preceptor/clinical facilitator at both formative and summative stages of the clinical placement.

You are required to submit your formative self-assessment and your assessor’s formative assessment at the midway point of both your clinical placements.

At the end of each of your clinical placements, you are required to submit your completed 2019 Clinical Placement Australian Nursing Standards Assessment tool (ANSAT) documentation, including your assessor’s summative assessment, your signed record of attendance and if applicable, your completed and signed support plan. These documents need to be uploaded to the course Moodle site within seven days of completing each clinical placement.

Students are required to meet the Minimum Expectations for Clinical Placement criteria (for both placements). A copy of the minimum expectations criteria is available on the NURS12158 Moodle site.

IMPORTANT – If you are identified as not achieving the required Minimum Expectations, standards, or competency for NURS12158, you may be required to complete further learning as outlined in a Learning Support Plan. The unit coordinator in consultation with the student and clinical facilitator/preceptor will prepare the individual learning support plan. If you are still not able to achieve the required standards or competency level with the assistance and direction of the learning support plan a fail grade will be awarded for NURS12158 Clinical Nursing Practice 3.

Unsatisfactory behaviours which jeopardise the safety of people in your care or that do not adhere with the NMBA Registered Nurse Standards for Practice, Code of Professional Conduct, Code of Ethics, Professional Boundaries and/or Social Media Policy criteria may result in you being removed from placement. If you do not meet each standard at a pass level you will receive a fail for the Unit.

Students who fail to meet the CQUniversity Student Code of Conduct maybe withdrawn from the clinical setting and a fail grade awarded.

Important: It is vital you refer to the semester 2, 2019 Australian Nursing Standards Assessment tool (ANSAT) available on the clinical placement Moodle site prior to and throughout your clinical placement.

To achieve a pass for this assessment item, you are required to access various online assessment portals (as directed below) to complete six professional practice activities and then upload evidence of a pass for each. The compilation of these certificates will then be referred to as your professional portfolio and will support your suitability for clinical practice as a student nurse.

Your task:

· Include a certificate for each activity achieved

· These certificates must have been completed within the last 12months. If you have completed these within the 12 months, these can be uploaded. If you have NOT completed these certificates within the last 12 months you MUST complete these certificates.
